高雄醫學大學醫學院學生參與校外英語能力檢定考試鼓勵方案
Encouraging Program for Medical Students to Participate in Off-campus English Proficiency Test
( 112學年度適用 )
※因應學校決策，本方案將於112-2學期末結束申請
一、目的(Activity purpose)：
為提升本學院學生英語能力，適應全英語授課，鼓勵學生參與校外英語能力檢定考試，訂定本方案。
To enhance the English proficiency of students in our college, adapt to full English instruction, and encourage students to participate in external English proficiency examinations, this plan is formulated.
二、鼓勵方式 (Implementation modalities)：
（一）凡本學院學生於在學期間參加校外英語能力檢定考試成績達該系所學程英文畢業門檻者，得補助其報名費，每人在學期間補助乙次為限。
Students of this college who achieve the English proficiency graduation threshold for their department's curriculum by participating in external English proficiency examinations during the academic semester can apply for a subsidy for the examination registration fee, with a limit of one application.
（二）考試成績高於該系所學程英文畢業門檻一級者，發予獎勵金300元。高於二級者發予獎勵金600元，高於三級者發予獎勵金1000元，最高補助獎
勵以高於三級為限。
Achieving a score higher than the departmental English graduation threshold by one level will be rewarded with a prize of NT$300. Exceeding it by two levels will result in a reward of NT$600, and exceeding it by three levels will merit a reward of NT$1000. The subsidy will be capped at reaching the three-level threshold.
（三）經濟弱勢學生除上述獎勵金之外，另加給新台幣 300 元之獎勵金。
Economically disadvantaged students will receive an additional reward of NT$300.
三、鼓勵對象 (Eligible participants)：
凡本學院學生於在學期間參加校外英語能力檢定考試並取得證明文件者，得於參加考試當學年度提出申請，逾期申請者不予受理。
Students of this college who participate in external English proficiency examinations during the academic semester and obtain proof of their performance can apply within the same academic year in which they took the exam. Applications submitted after the deadline will not be accepted.
四、申請作業 (Directions)：
（一）申請期間至113年5月31日止。 Application period is until May 31, 2024.
◎考試日期須為2023.08.01起 The examination dates need to start from August 1, 2023.
（二）學生申請時應先完成線上登錄作業(Google表單)，上傳檢定證書或成績單至英檢學生證照維護系統(D.2.5.05.證照(含英檢)維護)，並備妥下列文件，送至醫學院「大專校院學生雙語化學習計畫」學生獎勵小組申請。
At the time of application, please start by completing the online registration process and uploading the examination certificate or transcript to the Wac system (D.2.5.05). Then, prepare the following application documents and submit them to the personnel in charge at Room IR437, 4th floor of the Medical Research Building.

2.醫學院校外英語能力檢定考試鼓勵方案繳交資料檢核表。 *附件下載列印
Print the 112_Application Checklist.
3.報名校外英語能力檢定考試收據正本。
Copy of the examination registration fee.
4.學生證影本 (須檢附正本供查驗)。
Copy of student ID card.
5.學生銀行帳戶 (上傳至學生wac系統D.6.01銀行帳號維護 或 列印封面影本)。
Bank account. (Print a copy or upload a file at D.6.01)
6.校外英語能力檢定證書或成績單影本(須檢附正本供查驗)。
Copy of exam transcript or certificate.
（三）經濟弱勢證明(非必要)：低（中低）收入戶學生需檢附鄉鎮區公所核發有效期限內之低（中低）收入戶證明影本，需有學生姓名。
Economic hardship proof (optional)：Label the student’s name.
（四）若校外英語能力檢定證書或成績單上所載姓名為英文者，應另檢附足資證明英文姓名之證件(如護照等）以供驗證。
If the name on the certificate or transcript is in English, an additional document proving the English name (e.g., passport.) is required for verification.
（五）申請案件經醫學院「大專校院學生雙語化學習計畫」學生獎勵小組行政人員初審無誤，再經EMI學生獎勵小組通過後，經院方核定後核發。
After submitting all the documents, please await approval. You will be notified by mail once the review process is complete.
五、其他外語測驗及翻譯能力考試之獎助以專案處理。
Subsidies for other foreign language proficiency and translation ability tests will be subject to separate project-based evaluations.
六、經費來源由本學院教育部「大專校院學生雙語化學習計畫」支應，鼓勵方式得依每學年度計畫預算調整之。
Funding is provided by the Ministry of Education's project for this college and is adjusted according to the annual plan.
